Understanding ACA reporting
The Affordable Care Act (ACA) requires certain employers to report health coverage information to the Internal Revenue Service (IRS). Employers use Forms 1094-C and 1095-C to meet these reporting requirements.
What Form 1094-C reports
Form 1094-C serves as the transmittal form that employers submit to the IRS. It reports:
- Employer identification information.
- Total employee counts.
- Information about offered health coverage.
- Aggregated employer group details, when applicable.
- Certification of eligibility information.
What Form 1095-C reports
Form 1095-C provides employee-level health coverage information. It reports:
- Employee identification information.
- Months health coverage was offered.
- Details about employee coverage eligibility.
- Lowest-cost monthly premium information.
- Covered individual information, when required.
Who must file these forms
Applicable Large Employers (ALEs) must file Forms 1094-C and 1095-C. An ALE generally employs an average of 50 or more full-time employees, including full-time equivalent employees.
Filing deadlines
Employers must:
- Provide Form 1095-C to eligible employees by the required annual deadline.
- Submit Form 1094-C and Form 1095-C to the IRS by the applicable filing deadline.
- Follow IRS electronic filing requirements when they apply.
