You may complete the Employee Declaration form attached to Sage Knowledgebase KB article 19652 (see Related Resources section), fax or e-mail to Sage 100 Customer Support.

Perform the following:

  1. If you have a current support agreement, and are a valid contact for your account number, fill out the Employee Declaration Form.
  2. Support Ticket: We prefer that you first create a support ticket on-line, or call in to create a support ticket. (If calling: the number is (800) 854-3415. Please call on a weekday between 6:00 AM and 5:00 PM Pacific Time, select Customer Support). A form without a support ticket referenced on it may take much longer to process.
  3. Fax the completed form to 800-700-0417, and include your Account Number and Ticket Number on the fax cover sheet. Or, scan the completed form and e-mail it to [email protected], and include your Ticket Number in the e-mail subject line.
