Customizing the Quarterly Governmental Report:
The Quarterly Governmental Report default values print the current version of the Quarterly Governmental Report form used for quarterly reporting of income and tax information. If you need to add form codes for specific governmental tax and income reports, you can modify the existing format to meet your reporting requirements.
Note: If your state requires double spacing for this report, enter 002 in the Number of lines per detail item field on the Header tab.
The following table indicates what effect the field description has on quarterly report printing.
Field Description | Effect on Report |
Social Security Number | These fields print on the line specified within each employee line grouping. |
Name - Last Name First | These fields allow you to enter Y in the Misc column to print only the first initial of the first name or N to print the entire first name. |
Unemployment WGS To Limit | This field prints on the line specified within each employee line grouping. If you enter Y to print this field, only states where an employee has unemployment wages print. If you enter N in this field, information for all states where employees have wages print. |
Pension Plan Deductions | This field allows you to print pension plan deductions for each employee. |
Employee First Name | This field allows you to enter Y in the Misc column to print only the first initial of the first name or N to print the entire first name. |
Employee Hire Date | This field prints the employee hire date only if this date falls within the quarter for which you are printing the Quarterly Governmental Report. |
Detail Options:
By default, the report will print Unemployment wages TO LIMIT.
Note: You can also print the Quarterly Tax Report (under Payroll > Reports menu) to get the Taxable Unemployment Wages to limit
To print QTD Unemployment Wages pre-limit (total unemployment wages without the Unemployment limit applied) ( which is the Gross wages - any applicable deductions that are setup to reduce Unemployment wages):
If you do not get desired results, confirm none of the print lines are using the same tab settings.

Use the Quarterly Governmental Report for reporting quarterly income and tax information to various governmental agencies. The default format supplied with your Payroll module, includes quarterly Gross Wages and taxable Unemployment Wages (FUTA/SUTA).
If your company requires Quarterly Governmental reports to be filed electronically, you can use Federal or State eFiling and Reporting.
Note Employees with negative wage amounts for the quarter are not included on this report.
